First Office Visit

Save time by completing forms to bring with you before you arrive.

Please note that consultations and minor extraction appointments generally book 3 months in advance.

 

We are asking all of our patients to wait until their appointment time to enter the office. Walk-ins are not permitted. Please call to schedule an appointment. Please do not bring in guests or additional family members. We are trying to limit the number of people in the waiting room.

There are some procedures that require a consultation prior to surgery even with a referral i.e wisdom teeth removal, implants, grafting, biopsy, deep sedation, etc.

If your dentist or physician has taken x-rays, you may request that they forward them by mail or email to our office. Even though you may have recent routine dental films, we frequently need to take a special x-ray called a panorex to more carefully evaluate your teeth and the supporting jawbone. If additional films are necessary, we will take them at our facility.

Please assist us by providing the following information at the time of your appointment:

  1. Your surgical referral slip (i.e. paper with teeth circled etc.) and any X-rays if they were given to you. Have your dentist email them to us before your appointment date if you no longer have them.

  2. A list of medications you are presently taking.

  3. Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the their visit.
